Model 12A feeding problem... help, please

I have a Model 12A I just picked up a couple of weeks ago. It has a feeding problem in that every shell is high of the chamber, when you are trying to close the action. I can reach in with a pocket knife and move them down to line up, and then the action closes properly and the gun fires with no problems. Ejection is OK, too. I have tried several brands of shells and experienced the same problem. I am not that familiar with the 12A, having just picked this one up. It looks like the carrier is lifting the shell too high to feed properly. Is this a common problem and if so, how do I remedy it?

I've had two mod. 12's that did the same thing. I never figured out what was wrong with the first one but the 2nd one I repaired by changing the breach block (bolt). It seems that the slot that the shell slides up in wears out allowing the shell to tip up. I could be wrong in this but a breech block replacement fixed the problem for me.
