It's an early Model 11. The buttstock is unusual being straight grip, I've not seen that version.

There is a Model 1100 but it is a much newer model that the pictured.

I don't know the year but it is much newer than 1964, it does look like a Remington product.

There should be a small gap between the front of the buttstock and the receiver. The older Remingtons used a Stock Bearing Plate(SBP) to create this gap, the gap prevents chipping of wood during recoil ...

Some of the Nylon stocks, any model, can split along the seam. I would inspect the lengths of the seams for any separation.

If the spring still has strength then the damage may be cosmetic. If the spring is dead then any heat treat is probably lost, this could make it unsafe.

If the butt is flat then a recoil pad may have been added in the past, what you call "original plate" might be the remnants of the hard portion of the recoil pad after the soft portion has been removed.

I would be tempted to put a Pachmayr RP200 recoil pad on it instead of a plate. Pachmayr makes ...

I'm drawing a blank on those dims. I may have a couple books that will help if I can locate them. If the gun can be identified that will help. If the barrel has been changed or set back then it's a possibility that the cartridge is a wildcat.
