Your M/12 was made in 1911, long before Rem stamped the Model # on the receiver. The model stamp on the barrel was phased in between 1910 & 1917 so yours would be proper with or without the barrel model stamp.
It's very unusual that the buttplate # would be a match for the gun. The buttplate stamp on my earliest (serial #26) was off by ten(36). Many later ones are off by hundreds.
Many possibilities exist. The barrel could have been replaced. It could have come from the factory as it is now. Most 12-As with the stock you show had wood or the barrel replaced. In any case, it holds a little more collector interest if it's original. But not a whole bunch...

If its a 12-C with a barrel added then they also would have replaced the magazine tube as well. The 12-C tube would be longer. I have several 12-As with the straight stock crescent butt plate. It was an option that could be ordered, but it was seldomly ordered.
