Another Model 12-A Sight Questions

My Model 12-A has a flip-up tang site (2 screws) and a front sight. The rear slot is empty. I've seen some 12's with all three sights. Is there a "correct" configuration?
Is the sight a Lyman R-12? Next: Should the sight have an eyepiece of some sort or a simple peep with a slight elevation adjuster? Y'think this sight is original or were most of them added-on?

That sight is correct. It was a factory option, but most were added later. Lyman made two types; one had a removable aperture, the other had a flip up aperture design providing larger and smaller viewing. I don't know if the normal rear sight was included If the peep was ordered from the factory.
