Found in my granddad's gun cabinet

Can anyone help me identify any specifics on this model 12 I found in my granddad's gun cabinet? He died back in 2001, unfortunately I didn't have any interest in guns at the time so we never discussed them. However, I'd love to know all I can about it today. Unfortunately, having a brain tumor makes trying to sort through many search results rather difficult. Thanks in advance, it's a model 12 the serial number is 667823. I've attached several photos with more markings I've yet to figure

Made in 1926. Could likely find a piece of lumber for the forend without a whole lot of trouble

Your M/12 was made in 1926. The forearm is missing.
